At Treat we understand that you have busy lives and might need to cancel appointments. To allow for efficient treatment scheduling and to ensure that you are cared for in the best possible manner, we ask you to:
• Please arrive or be at the location of the service about 15 minutes before your appointment to allow yourself time to get settled at a leisurely pace. • For your convenience, you can print out our mandatory first-visit intake form. Pssssst....
We strive to make the Treat experience as relaxing as possible for you. Therefore we kindly ask you to turn off or silence all pagers and cell phones in order to keep a serene environment.
Cancellation Policy
An appointment with a therapist is a commitment on your part to purchase that therapist's time, and other resources. This commitment is in effect unless appropriate cancellation of the appointment is made by you. When you make an appointment for one of our services, we reserve that time, and a therapist to provide you with those services. We do not sell those resources to anyone else because you have committed to purchasing those resources.
We require credit card information in order to hold a scheduled appointment. If you are unable to make your appointment for any reason, we ask you give at least 24 hours notice to cancel or reschedule. Failure to do so will result with mandatory billing of service in full. If a gift certificate was to be used, the amount of service to be rendered will be deducted from the certificate. We do not charge any cancellation fees for appointment cancellations made 24 hours prior to the scheduled appointment. If you do not cancel within 24-hours prior to the appointment, we will charge you the full fee. For rescheduled appointments we require a 50% deposit. We realize that there may be good reasons why you couldn't cancel and we may waive the fee in certain cases as long as the appointment is made and kept within three days of the original appointment. Late Fee Assessments Clients who are up to 15 minutes late for appointments will be charged full session price, but the session may end at the same time that the scheduled appointment would have ended had the session started on time. Clients who are 15 to 30 minutes late will be charged a full session price but may receive only a thirty minute session. If the client is more than thirty minutes late the appointment may be canceled altogether. In this instance, the cancellation fee schedule applies. No Show Full session fees will be assessed for scheduled appointments whenever the client does not show up for the appointment. No credit shall be given under these circumstances. |
We want to assure our clients that discussions and medical information exchanged between the massage therapist and the client is absolutely confidential. While we consider this a normal standard of practise, we are furthermore required to do so under the HIPPA laws.
Draping
For the massage modalities that are applied to an undressed body you will be draped at all times with sheets and towels. Only the body part being worked on will be undraped.
Existing/New Medical Conditions
It is the responsibility of the client to keep the massage therapist informed of any medical treatment currently being taken, and to provide written permission from the physician, chiropractor, physical therapist, etc., that the massage may be continued. The client must also keep the massage therapist informed of any changes in health conditions.
Gift Certificates
Gift Certificates are non-refundable and must be presented at time of service. Lost or stolen certificates are also non-refundable and are invalid.
Informed Consent
The intake session provides time for the massage therapist to discuss each section of the massage therapy policies contained in this website. Prior to each massage session, the treatment plan will be discussed with you. At your first visit with us you will receive a copy of the massage therapy policies and will be asked to sign the consent stating that you have read the information, understand it, and agree to comply with the professional massage therapy policies and procedures.
Harassment
Under no circumstances will any Treat therapist perform any kind of massage even remotely associated with sexual massage, ever.
Any comments and/or behaviors that are sexually inappropriate will prompt the Treat therapist to immediately terminate the session and charge in full for that appointment. Treat Therapists who are approached by clients requesting sexual contact of any kind, or experience a client trying to touch them in any inappropriate manner shall end the session immediately and ask the client to dress, pay for the session and to immediately leave the premises. Therapists shall leave the area and will not return until the client has departed. The client's name will be entered into the prohibited client database to prevent any subsequent appointments being made by the client. |
